Description

Amaryl M is an oral medication used in the treatment of non-insulin-dependent diabetes mellitus (NIDDM). This medication is usually used when monotherapy did not work out for a patient.

Amaryl M contains two active ingredients with antihyperglycemic properties that have different mechanisms of action. Glimepiride, a sulfonylurea agent, decreases blood glucose levels by stimulating the release of insulin from the pancreas and by improving insulin-response of peripheral tissues. Metformin, a biguanide, not only reduces the production of glucose by the liver but also slows down glucose absorption by the intestine and improves insulin sensitivity.

To achieve a proper glycemic control the treatment with Amaryl M has to be combined with a healthy lifestyle and regular physical activity.

Indications

Treatment of type 2 (non-insulin-dependent) diabetes mellitus in patients who cannot achieve a proper glycemic control with a single hypoglycemic agent.

MOA (mechanism of action)

Amaryl M contains two antihyperglycemic agents with different mechanisms of action that complement each other: Glimepiride and Metformin.

Glimepiride is a sulfonylurea that belongs to insulin secretagogues and it acts by blocking ATP-sensitive potassium channels which leads to depolarization, insulin exocytosis, and calcium ion influx. This promotes insulin release from functioning pancreatic beta cells and enhances insulin-response of peripheral tissues.

Metformin’s mode of action is different from other anti-diabetic drugs and it can work in 3 ways. By activating AMP-activated protein kinase (AMPK) it reinforces the reduction of glucose production in the liver. Its ability to increase peripheral utilization and uptake of glucose leads to a decrease of glucose absorbed by the intestine and improved insulin sensitivity.

Dosage

Amaryl M is taken orally during or after the first meal of the day and should be swallowed whole with some water.

The dosage of the medication should be adjusted individually. Consult your doctor before taking Amaryl M.

Maximum recommended dose: 8 mg Glimepiride/2000 mg Metformin a day.

Missed dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a forgotten one.

Overdose

An excessive dose of Amaryl M can lead to lactic acidosis or hypoglycemia.

If you are experiencing symptoms of hypoglycemia, you should raise your blood sugar right away by consuming oral glucose (e.g., a small bar of sugar cubes, sweet juice, sweetened tea).

If you are experiencing some of the lactic acidosis symptoms (which include vomiting, abdominal pain, muscle cramps, severe tiredness, feeling sick, difficulty in breathing, reduced heartbeat etc.), contact your healthcare provider immediately, as this condition may lead to a coma.

Storage

Store at room temperature between 20°C and 25°C (68°F and 77°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

The most common side effects associated with Amaryl M are:

  • hypoglycemia
  • lactic acidosis
  • nausea
  • diarrhea
  • abdominal pain
  • constipation
  • vomiting
  • metallic taste in the mouth
  • skin reactions (e.g., rash, pruritus, urticaria, erythema, flushing)
  • headache
  • dizziness
  • leukopenia
  • agranulocytosis
  • thrombocytopenia
  • hemolytic or aplastic anemia
  • pancytopenia
  • hepatic porphyria reactions
  • low levels of vitamin B12

Side effect occurrence does not only depend on medication you are taking but also on your overall health and other factors.

Contraindications

Do not take Amaryl M if:

  • you are allergic to Glimepiride or Metformin or any other ingredients in the formulation;
  • you have a severe hepatic or renal impairment;
  • you suffer from any metabolic acidosis, including diabetic ketoacidosis;
  • you are going to undergo surgery;
  • you are going to have an injection of a dye or a contrast agent for an X-ray imaging;
  • you are suffering from dehydration, severe infection, or shock;
  • you have a history of heart diseases or glucose 6-phosphate dehydrogenase (G6PD) deficiency;
  • you are pregnant or breastfeeding;
  • you are under the age of 18.

Co-administration of Amaryl M with some other medicines may lead to hypoglycemia, i.e.:

  • insulin
  • other oral anti-diabetics
  • ACE inhibitors
  • allopurinol
  • anabolic steroids and male-sex-hormones
  • chloramphenicol
  • coumarin derivatives
  • cyclophosphamide
  • disopyramide
  • fenfluramine
  • fenyramidol
  • fibrates
  • fluoxetine
  • guanethidine
  • ifosfamide
  • MAO-inhibitors
  • miconazole
  • para-aminosalicylic acid
  • pentoxifylline (high dose parenteral)
  • phenylbutazone
  • azapropazone
  • oxyphenbutazone
  • probenecid
  • quinolones
  • salicylates
  • sulfinpyrazone
  • sulfonamides
  • tetracyclines
  • tritoqualine
  • trofosfamide

Thyroid hormones may weaken the blood-lowering effect of Amaryl M.

Caution and close monitoring are needed while taking of Amaryl M with any of the following:

  • phenylbutazone
  • furosemide
  • nifedipine
  • danazol
  • thiazide
  • angiotensin enzyme inhibitors (captopril, enalapril)
  • other anti-diabetic drugs
  • beta-blockers
  • fluconazole
  • monoamine oxidase inhibitors (MAOIs)
  • sulphonamide
  • NSAIDs

Consult your doctor before starting the treatment with Amaryl M if you’re taking cationic medications that are eliminated by renal tubular secretion.
